Treatment and Care Coordinator / Nurse
The Jamaica Country Coordinating Mechanism for HIV/AIDS has received a three (3) year (January 2016 – December 2018) grant from the Global Fund to fight AIDS, Tuberculosis and Malaria. This grant will be managed by the Ministry of Health, the Principal Recipient which has contracted the Jamaica AIDS Support for Life (JASL) as one of its implementing entities. JASL and its sub-sub-recipients are seeking dedicated, dynamic, proactive and experienced individuals with strong team building skills to fill the following position: Treatment and Care Coordinator / Nurse
Summary of Duties
The Treatment and Care Coordinator /Nurse will be responsible for the management and execution of efficient nursing care and treatment of patients, specifically persons infected and affected with HIV/AIDS. He/She will also be responsible for the management of the support services programme specifically all activities within the clinic.
Qualifications
- Bachelor’s Degree in Nursing from a Recognized Institution
- Certified District Nurse
- Should be fully registered with Nursing Council of Jamaica as a EAN or Registered Nurse (RN) with Biennial renewal
Experience
- Minimum of five (5) years active nursing experience
- Two (2) years’ experience in HIV management
- Two (2) years’ experience working with MARPS population
Specific Deliverables
Clinical
- Registration, Triage, and screening of Patients
- Monitoring of patients via mechanical devices and takes appropriate action (glucometer, blood pressure)
- Conduct needs assessment and provide necessary referrals within JASL (Doctor, psychologist, support groups, Nutritionist, VCT )
- Education and Counselling of Patients in the following areas (HIV/STI, Chronic Illness, Acute infections, Treatment Failure)
- Provide general counselling
- Conduct Home Base Care with clients who are unable to visit
- Conducts visit to patients who are hospitalized
- Ensure the proper collection, identification, storage and safe transportation of specimens to the laboratory with the appropriate requisition forms; Receive and record results
- Provide Assessment of medical needs and referral to appropriate agencies (CHARES, Hospices, Government clinics)
- Forecast, plan, implement, coordinate and evaluate nursing care for patients
- Implement and maintain an effective system to link and retain patients
- Provide consultation and support to necessary departments regarding practice changes that affect patient care
- To create and or share documents that promote best practice, such as clinical
- practice guidelines, systematic reviews, journal articles, etc
- Keep current about trends and changes relating to HIV/AIDS prevention, treatment and care.
- Manage Client Records.
Administrative
- Liase with other clinical staff (Doctor, Nurse, Pharmacist, Nutritionist)
- 2. Coordinate sessional clinics with Doctor and other Health Care Personnel
- (Counsellor, Dietician/Nutritionist, Social Worker)
- Collaborate with Medical Doctor regarding Planning Patient care
- Develop care plan for each client
- Management of data base
- Facilitate support group sessions with PLHIV and other JASL clients
- Facilitate and coordinate treatment literacy
- Assist with programme development and proposal writing
- Develop strategies to resolve issues, make recommendations and support implementations where appropriate
- Represents JASL locally at Ministries, seminar sand meetings, Funding Agencies, Conferences, Public events
- Submit monthly reports
- Perform other relevant Duties as assigned
